From your first study plan to your first week at university
If you are thinking about studying medicine in Italy in 2027, you probably have more questions than answers. What is IMAT? Can an Indian student apply after Class 12? How much money will your family need? When should you prepare documents? And after getting a seat, when can you actually travel?
These questions belong together. Preparing for an entrance examination is only one part of moving abroad for a medical degree. You also need to choose a suitable university, follow the right application process, organise funding and arrive before your university’s deadlines.
This EuroPick Education guide walks through that journey in simple language. It is written mainly for Indian students applying from outside the European Union, although much of the preparation advice will help other international applicants too.
A note about 2027 dates: we did not find a confirmed IMAT 2027 examination calendar in the official sources checked for this article. Exam dates, registration windows, seats and university deadlines must be checked against the 2027 admission notices. However, the published Universitaly international-student procedures already cover 2027–28.
1 What IMAT is and where it fits
IMAT stands for International Medical Admissions Test. It is the entrance-test route used for participating English-taught medical programmes in Italy. The annual admission notice explains which courses participate, who can apply and how places are allocated.
When families search for “MBBS in Italy,” they are usually looking for the Italian Medicine and Surgery degree. Its formal title differs from the Indian MBBS title. For example, Bologna lists its English-taught Medicine and Surgery programme as a six-year single-cycle degree. “Single cycle” means one continuous degree rather than a separate bachelor’s followed by a separate master’s. Always check the exact course title, teaching language and campus. A university can offer more than one medical programme, and similarly named courses may follow different admission procedures.
Do not assume every private medical university uses IMAT. Likewise, news about changes to Italian-language medical admissions should not automatically be applied to English-taught courses. Read the notice for the programme you actually want.
2 Decide whether Italy suits your plans
Start with the life you are choosing, not just the entrance exam. You will be studying a demanding subject while learning to manage accommodation, money, paperwork and daily life away from home.
Ask yourself whether you enjoy biology and human physiology enough to study them beyond the school syllabus. Consider how you respond when a concept takes several attempts to understand. Medicine requires patience with uncertainty, sustained study and responsibility towards other people.
Discuss the financial plan with your family before paying application or counselling fees. Decide who will fund the first year if scholarship payments are delayed or no award is received. Think about where you might want to practise after graduation, because that choice affects the checks you must make before enrolling.
You do not need every future decision settled today. You do need a realistic reason for choosing the course. “I want medical training in an international setting and I understand the work involved” is a stronger foundation than choosing a country only because a poster says it is free.
3 Check your academic eligibility first
A typical applicant needs a recognised school-leaving qualification that meets Italian university entry requirements. Bologna’s published guidance includes foreign qualifications following at least twelve years of schooling. For an Indian applicant, collect your Class 10 and Class 12 records and write down your board, subjects, passing years and any additional qualifications. Do not treat “I completed Class 12” as a complete eligibility assessment.
If you changed streams, added science subjects later, studied through open schooling or completed qualifications across different boards, send the full history to the university. Request a written answer about your specific combination. The same approach applies to students with fewer than twelve school years or qualifications completed across different countries.
If your final results are pending, ask whether an application with provisional documents is permitted and when final certificates must arrive. A portal accepting an upload does not, by itself, mean the qualification has been approved.
4 Understand NEET and future registration in India
Indian applicants should plan for NEET alongside their Italian admission requirements. The Ministry of External Affairs advises students pursuing medical studies abroad that NEET-UG qualification is mandatory under the applicable Indian framework. IMAT and NEET serve different purposes; one should not be treated as a substitute for the other. MEA advisory for medical students.
Before committing to a university, check the NMC rules applicable to your admission year. Ask about the required course duration, subjects, teaching medium, internship, clinical training and eligibility for registration in the country awarding the degree. Obtain written programme information from the university. Ask how clinical placements and practical training are recorded, what certificates graduates receive and what additional steps are required to practise locally.
Do not assume that any six-year foreign medical degree automatically meets every Indian requirement. Nor should you assume that the examination system operating today will remain unchanged when you graduate. Keep your curriculum, transcripts and training records carefully throughout the course.
5 Identify your applicant category
Your applicant category affects which places and ranking procedures apply to you. An Indian citizen living in India will generally need to examine the rules for non-EU applicants residing abroad. An applicant already living in Italy may have a different position, depending on their legal status and the admission rules.
Citizenship alone is not always enough to resolve the question. Equally, having a visa, a relative in Europe or permission to visit another European country does not automatically place you in the EU-equivalent category.
Read the university’s definition and ask its admissions office if your situation is unusual. Explain your nationality, country of residence and residence-document type without assuming the answer.
For university planning, use the seats and ranking information for your own category. A large total intake can be misleading if only part of it is available to applicants in your group. Keep a copy of any written clarification you receive. Bologna admission guidance.
6 Understand the exam format before studying
Use the latest confirmed format as your preparation baseline, then adjust when the 2027 rules appear. Bologna’s published 2026–27 description specifies sixty multiple-choice questions, each with five options and one answer to select. Its subject distribution is shown below. This is a reference for planning, not a confirmed 2027 structure. Published exam structure.

Recent IMAT formats have used 100 minutes and scoring of 1.5 for a correct answer, minus 0.4 for an incorrect answer and zero for an unanswered question. Reconfirm these details in the 2027 notice before setting your final mock-test conditions. IMAT format and scoring reference.
Under that scoring system, forty correct answers and ten incorrect answers produce 56 points: 60 minus 4. Ten unanswered questions add nothing. This is why accuracy and time management belong in your preparation from the beginning.
7 Build a clear syllabus map
Divide your preparation into topic groups. For biology, a useful starting map includes cells, biological molecules, enzymes, genetics, molecular biology, human physiology, reproduction and evolution. For chemistry, organise atomic structure, bonding, mole calculations, solutions, equilibrium, acids and bases, redox and organic chemistry.
For physics, work through units, mechanics, fluids, thermal physics, electricity and other topics included in the official syllabus. For mathematics, review algebra, equations, inequalities, functions, graphs, geometry, trigonometry, probability and basic statistics where required.
Treat this paragraph as a study organiser, not a replacement for the official syllabus. When the 2027 syllabus is available, compare it line by line with your notes. Mark each topic as unfamiliar, partly understood or ready for timed practice.
Do the same for reasoning and reading. These areas deserve regular attention even when their question count is smaller. A few avoidable mistakes can matter in a competitive ranking.
Keep one main checklist. Collecting five different syllabus screenshots usually creates more confusion than progress.
8 Start with a diagnostic test
Before designing an ambitious timetable, find out where you are starting. Attempt a suitable past paper or representative test under timed conditions. If much of the syllabus is new, the result may be low. That is useful information, not a prediction of your final performance.
For every incorrect answer, identify the reason. Did you lack the concept, forget a fact, misread a word, make an arithmetic error or run out of time? Each problem needs a different response.
For example, rereading an entire genetics chapter will not solve a habit of overlooking “except” in the question. Similarly, doing more timed questions will not repair a chemistry concept you have never understood.
Create an error notebook with the topic, your mistake and the correct reasoning. Keep explanations short enough to revise. Once a week, reattempt selected questions without looking at the answers.
Your first goal is to make fewer repeated mistakes. A rising score becomes more meaningful when you can explain what improved.
9 Follow a preparation routine you can sustain
If you are starting in late 2026, use the early months to build foundations and establish a routine. Students managing school or NEET preparation should adapt their workload rather than copy someone else’s ten-hour timetable.
A practical study session might begin with one concept, continue with a small question set and finish with a review of mistakes. Leave time during the week for revision of older chapters. Without that return visit, a topic that felt easy in October can feel unfamiliar by February.
During the middle phase, combine subjects and introduce timed sections. Later, increase full-length practice and rehearse your exam strategy. Keep some unused papers for this final stage so that your scores are not inflated by remembering answers.
Use specific weekly goals: complete acid–base calculations, revise circulation and solve a timed algebra set. “Study chemistry harder” is difficult to measure.
Protect sleep and short breaks. If concentration falls sharply, adjust the routine. Consistency over several months is more useful than an exhausting week followed by a long break.
10 Use school knowledge and past papers wisely
Indian students can use their school science foundation, including relevant NCERT material, as a starting point. Then practise interpreting questions in English and applying familiar concepts in unfamiliar situations.
When reviewing a multiple-choice question, explain why the correct option works and why the alternatives fail. This reveals gaps that memorising the answer letter would hide. For calculation questions, check units, signs and whether the final value is physically reasonable.
Official past papers help you understand the style of questions. The MUR archive provides published IMAT papers. Note that the published 2025 version places the correct answer at A for every question, so simply memorising those option positions is not meaningful practice. Official IMAT paper archive.
Track accuracy by topic and the time spent on questions. If you repeatedly spend several minutes on one difficult calculation, practise deciding when to move on and return later.
Keep your resources manageable. One well-used question bank and an honest review process can be more useful than a folder full of untouched material.
11 Choose universities using more than a cutoff
Begin with a shortlist of programmes that meet your academic and career needs. Check their official course pages for teaching language, campus, curriculum and application instructions. Confirm participation and seats again when the 2027 notices appear.

Previous cutoffs are historical results, not a guaranteed target. They change with exam difficulty, seat numbers and applicant performance. Avoid choosing a university only because someone predicts it will be “easy” next year.
Speak to current students about daily life, but separate personal experience from official rules. Their experience can help you understand housing or transport; an admission deadline still needs an official source.
12 Create one calendar for the whole journey
There is no single application date covering every step. Build a calendar that separately tracks university applications, Universitaly pre-enrolment, IMAT registration, scholarship applications, document submission, enrolment, visas and arrival.

Do not copy general bachelor’s or master’s application windows into your medicine calendar. Record the source and year beside every deadline, including the time zone and whether submission or payment must be completed by that time.
13 Prepare your passport and personal documents
Start with your passport. Check the spelling of your name, date of birth and validity. If you need a new passport or a correction, beginning early gives you time to resolve problems without interrupting exam preparation.
Build a folder containing your school certificates, marksheets, identification records and any documents explaining a name difference. Add the NEET record and language evidence when relevant. Keep originals safe and make clear scans of complete pages.
Give files understandable names. “Class12_Marksheet_Name.pdf” is easier to identify than “scan_final_new2.pdf.” Check that pages are readable after compression and that signatures, seals and reverse-side information have not been cropped.
Use an email address that you control for applications. Keep access to recovery details and save payment receipts and submission confirmations. Counselling support should help you understand your application, while you retain access to your records.
Review every submitted form yourself. A small identity error can create repeated problems when several institutions compare the same documents.
14 Understand DOV CIMEA and document authentication
You may encounter a Declaration of Value, often called DOV, or qualification statements issued through CIMEA. These relate to understanding or verifying foreign qualifications. The university decides how it evaluates your qualification and what supporting evidence it requires. Do not automatically purchase every available service. Ask for the exact requirement: which document, for which qualification, from which authority and by what deadline? Also check whether the visa office has separate instructions.
Notarisation, apostille, legalisation and translation are different procedures. A notarised photocopy does not automatically replace an apostille or a required translation. For students educated across countries, ask the relevant Italian representation which office handles the qualification rather than deciding from citizenship alone. Before sending originals anywhere, keep copies and confirm the return process. Track appointments and processing time alongside your application deadlines.
15 Check English requirements and begin Italian
An English-taught course does not automatically mean every applicant is exempt from providing English evidence. Read the university’s rules on certificates, accepted tests, exemptions and deadlines. If you have a medium-of-instruction letter, ask whether that particular document is acceptable.
Start basic Italian while you prepare for IMAT. Ten or fifteen minutes a day can help you become comfortable with greetings, numbers, directions, appointments and simple questions. Later, add vocabulary for university offices and healthcare settings.
Patients may not speak English, even when your lectures do. Bologna’s published guidance, for example, describes a B1 Italian requirement for hospital-based clerkships from Year 2. That is a programme-specific example; verify the rule at your chosen university. Bologna language guidance.
Learning Italian also makes ordinary tasks less stressful. Reading a rental notice or explaining a problem at an office becomes easier when every interaction does not depend on translation.
You do not need fluency before your first IMAT lesson. You do need to take the language seriously.
16 Complete university applications and pre enrolment
Some programmes require university-level registration or document assessment in addition to the national examination process. Read the sequence carefully. Create the correct account, select the exact course and upload the requested documents.
For visa-seeking applicants, Universitaly is the official pre-enrolment portal. Pre-enrolment supports the study-visa process; it is not an automatic admission decision or visa guarantee. Official pre-enrolment procedures.
Follow your university’s instructions on when to submit and when validation takes place. Do not assume that every institution validates applications before the exam. Bologna’s current guidance describes validation after passing the entry exam and paying the required first instalment. That example shows why another university’s timeline cannot simply be copied. After submission, check for requests to correct or add documents. Save the application summary and record your status. If something is unclear, contact the relevant office with your application number and a precise question.
17 Register for IMAT separately
Completing pre-enrolment does not mean you have registered for IMAT. When the official 2027 examination window opens, follow the designated registration process and any additional university requirements.
Check your identity information, applicant category, university selection and test location before paying. Read the confirmation screen carefully. Keep the payment receipt and verify that the application shows as completed rather than merely saved.
Choose a test centre based on the official list for that year. A city that hosted IMAT previously may not necessarily be available in 2027. Check the venue instructions and any travel requirements before committing to a distant location.
If you need disability-related adjustments or other permitted arrangements, examine the application procedure early. Such requests can have separate deadlines and documentation requirements.
Set your own completion target ahead of the official closing date. This gives you time to deal with a failed payment or incorrect entry. Do not rely on an extension, and do not assume that making an account reserves a test place.
18 Prepare for the examination day
In the final weeks, practise the complete routine: timed questions, decisions about difficult items and careful recording of answers. Rehearse using only the materials allowed by the current examination rules.
Read the test-centre instructions again a few days before the exam. Confirm the address, reporting time, required identification and permitted items. If travelling to another country, separately verify its entry requirements. Permission to sit an exam is not permission to enter that country.
Where practical, arrive in the examination city early enough to avoid depending on a flight or long train journey on exam morning. Choose accommodation based on the actual venue, not just the city name.
On the day, read administrative instructions before beginning. If a question takes too long, mark it for review in the permitted way and continue. Leave time to check that your recorded answers match your intentions.
Afterwards, keep your registration and candidate records. Discussing answers with friends is understandable, but official results and ranking notices are what determine your next steps.
19 Follow results and enrolment instructions closely
A score and an admission offer are different things. Once results appear, check the ranking procedure for your category and university. Do not assume that the national ranking process and a non-EU university ranking have identical rules.
Read your status together with the action required. You may need to accept a place, pay an instalment, submit documents or complete identity checks by a short deadline. Put that deadline into your calendar immediately and keep evidence of completion.
If you are waiting for a possible vacant place, monitor the official notices. Do not assume another round will happen or that a place can be held informally. Send specific questions to the admissions office when the notice does not resolve your situation.
If you do not obtain a seat, review your options without rushing into an expensive alternative. Examine your score, remaining timelines and the full admission conditions of any backup course. A disappointing result deserves a considered next step, not a decision made under pressure.
20 Understand tuition and build a real budget
Public university tuition is not automatically zero for every international medical student. The amount can depend on the institution, economic documentation and applicable exemptions. Bologna, for example, publishes its own fee and exemption system. Official fee guidance.
Prepare a budget in euros before converting it into rupees. Separate recurring expenses from the money you need before departure. Ask for current quotations instead of treating an old social-media price as a 2027 guarantee.

For illustration only, if your own verified living-cost estimate is €800 per month, twelve months would be €9,600 before tuition and initial expenses. This is arithmetic, not an official cost-of-living estimate or visa threshold.
21 Apply for scholarships on their own timetable
Families often use “DSU scholarship” as a general label for Italian right-to-study support. The actual administering body, application portal and conditions depend on the university and region. There is no single application that automatically covers every medical school.
Read the relevant annual call for the benefits you want. Distinguish a scholarship payment from accommodation, meal services and tuition reductions. Ask whether separate applications or selections are required, and whether you can apply before completing university enrolment.
Do not wait for the IMAT result without checking this. Financial-support deadlines may operate independently of admission. Bologna explicitly advises applicants to prepare scholarship and economic documentation during the admission process. Funding guidance for applicants.
Keep a backup financial plan. A scholarship application is not an award, and an award does not mean all funds will be available before you travel. Read the payment schedule, housing conditions and academic-progress requirements in the call.
Your family should understand what would happen if you receive only part of the support you hoped for.
22 Organise family income and asset evidence
Scholarship and fee assessments can require documents about the family’s economic situation abroad. ER.GO’s guidance describes official documentation, translation and authentication requirements, with specified income and asset reference periods. Depending on the authority’s checklist, you may need evidence concerning family members, income, bank assets and property. Establish exactly what is required before requesting certificates. An income certificate alone may not answer every question on an application.
Ask which year the documents must describe and when they must be issued. Do not assume all regions use the same reference periods or the same ISEE procedure. Some processes use an equivalent assessment commonly called ISEE parificato; others require foreign economic documents through their own system.
Make one checklist with your family. Record who will obtain each document, the issuing office and the deadline. If a parent has no income or the family owns no property, ask how that situation must be documented rather than leaving it unexplained.
23 Prepare and submit your study visa application
Check the Italian embassy or consulate responsible for your place of legal residence and use its current study-visa checklist. For a course longer than ninety days, the relevant route is generally a national study visa. Bangalore’s official guidance directs applicants through admission, Universitaly pre-enrolment and then the appropriate visa process. Official consular guidance.
Prepare the file around the actual checklist. Common categories include your passport, application form, photographs, university and pre-enrolment records, financial evidence, accommodation and insurance. Keep financial evidence understandable. Explain sponsorship and the source of funds where required. If using a loan, deposit or scholarship, confirm what evidence the office accepts; do not assume the product’s name alone makes it sufficient.
Be ready to explain your course, university choice, funding and accommodation honestly. Submit promptly once you meet the required conditions. Bangalore warns that long-stay processing can take up to ninety days, with longer overall timelines possible when extra documents are requested. Avoid planning around a guaranteed quick decision.
24 Decide when you should reach university
There is no single date when all IMAT 2027 students must reach Italy. Your arrival depends on admission, enrolment, visa issuance, accommodation and your programme’s teaching and attendance rules.
Ask your university for four separate dates: the enrolment deadline, orientation date, first teaching day and latest permitted arrival. Also ask whether there are compulsory activities before regular lectures. Save the answer.
Once your visa and accommodation permit it, a practical target is to arrive around one or two weeks before required activities. This is a planning suggestion, not an official rule. It gives you time to find the campus, settle into your room and begin administrative tasks.
If your visa is delayed, contact the university before its deadline. Ask whether late arrival is allowed and what evidence it needs. Do not assume online attendance or an extension will be offered.
The currently published 31 October 2027 outer visa-submission deadline does not authorise you to miss earlier university requirements. Plan backwards from your own course calendar. Universitaly deadline rules.
25 Arrange housing and prepare to travel
Search for housing early, but investigate an offer before paying. Confirm the address, identity of the landlord or provider, contract terms, deposit, utilities and cancellation conditions. Ask whether the accommodation can provide the documentation required for your arrival procedures.
Check the journey from the room to the actual teaching campus. A cheaper room may become less convenient if it requires several connections, particularly for early classes. Temporary accommodation can provide breathing space while you assess longer-term options.
Before booking travel, check your visa details, permitted entry period, passport information and transit requirements. Choose a route you can manage with luggage and a realistic connection time.
Carry essential documents in hand luggage: passport, visa, university records, accommodation details, insurance and important contact information. Keep secure digital copies as well. Pack prescriptions for necessary medicines and check transport rules for them.
Arrange how you will reach your accommodation from the airport. A confirmed address, working payment method and charged phone make the first evening much easier.
26 Handle the first week in Italy
Contact the international office soon after arrival and confirm which enrolment steps remain. Ask how to activate your university account, access the timetable and obtain your student identification.
For a non-EU student requiring a study residence permit, begin the application promptly. The EU immigration guidance states that the application is required within eight days of arrival and describes submission through the post office followed by a Questura appointment. The initial submission does not mean the physical permit will be issued immediately. Keep the receipt and follow appointment instructions. Residence permit procedure.
Ask the university how to obtain or confirm your codice fiscale, the Italian tax identification code. Check healthcare arrangements and make sure there is no gap in your cover. Confirm any remaining scholarship, housing or bank-detail submissions.
Walk to your teaching building before your first class. Locate a supermarket, pharmacy and nearby transport stop. These small tasks help you settle while the bigger administrative processes continue.
27 Build a steady student life
Your first month may feel busy even before coursework becomes demanding. Give yourself time to adjust to new teaching styles, food, language and routines. Ask for help early when you cannot understand an administrative message.
Attend orientation and learn how exams are booked, where materials are posted and which activities require attendance. Keep a weekly study routine rather than leaving everything until an examination period.
Continue Italian practice. Build friendships beyond your immediate language group and learn from classmates without comparing every grade. Stay in touch with family, especially if they are helping manage money or documents from home.
If you are considering paid work, check the rules attached to your status and the demands of your course. Current EU guidance describes a twenty-hour weekly limit and a 1,040-hour annual ceiling for student employment. These limits do not guarantee that suitable work will be available. Set reminders for future permit, scholarship and fee deadlines. Good organisation should continue after you receive your first university identity card.
